I am considered an out of network provider. You will need to contact your insurance company to determine if you have out of network coverage, and if so, does the insurance company require a neuropsychological testing authorization form to be completed. If so, please download the authorization form that your company requires, complete the top portion with your information, and then I will need to interview you in order to complete the rest of the insurance authorization form. Since completing these forms can take much time, you will be charged a fee for this service.
